Mission Statement: Our mission is to provide outstanding personalized dental care with honesty, skill and compassion by offering every patient, treatment that is unquestionably in their best interest in a behaviorally sensitive and timely manner.
	Dr. Shirley Mathew, D.D.S, F.A.G.D. Dr. Shirley E. Mathew received her D.D.S. from the University of the Pacific, San Francisco, California. Dr. Mathew has been practicing in California and Texas and moved to this location in April 1998. Dr. Mathew cares about long-term dental health and demonstrate that concern by belonging to the American Dental Association, Academy of General Dentistry, Texas Dental Association, and Dallas County Dental Society. Dr. Mathew has also just been awarded the Fellowship of Academy of General Dentistry. Dr. Mathew participates in an ongoing program of professional development and continuing education to remain current with advances in the profession and to provide quality patient treatment. When not at work Dr. Mathew actively participate in church activities and enjoy working with children. She is also a Sunday School teacher.
	Huong Thi Nguyen Huong Nguyen graduated from Lousiana State University and is the Registered Dental Hygienist for Creekview Family Dentistry. Our patients show the most gratitude to her care, attentiveness and caring nature. Her favorite aspect of her job is seeing her patients smile. Ms. Nguyen enjoys being active, reading and spending quality time with her close family and friends.
	Jacqueline Chavez Jacqueline was born and raised in the Dallas area and has worked in the dental field for a while. She graduated from R.L. Turner High School. Her favorite aspect about working in the dental industry is meeting new patients and making sure everyone leaves with a smile on their face. In her spare time, she enjoys reading, relaxing, and spending time with her loved ones.
	Katherine "Kasia" Olszewski Kasia was born in Poland and raised in New Jersey and has been in the Dallas area for three years. She earned her degree from Bridgeton County College in South Jersey and has been in the dental industry for 13 years. Her favorite aspect of her job is interacting with the patients and making them smile. In her spare time, she enjoys watching movies and concerts, cooking, reading, travel, swimming, and participating in the Junior League of Collin County. When at home, she takes care of her dog, Lucy, and her cat, Deion.
	Lourdes Linares Lourdes was born and raised in Caracas, Venezuela and has lived in the Dallas area for 6 years. She earned her degree from Collin County Community College and has been in dentistry for 4 years. Her favorite aspect of the job is meeting and talking to new people and assisting them in caring for their oral health. In her spare time, she enjoys playing disc golf, hiking, and taking care of her cat, Cassie.
	Libby Vadakedom Libby was born in New York, raised in Texas, and has been in the Dallas area for 11 years. She recently earned her master's degree in Healthcare Management from the University of Texas at Dallas. Her favorite aspects of the dental industry are interacting with the patients and working with her coworkers. In her spare time, she teaches Sunday School, reads, watches movies, listens to music, and hangs out with her family and friends.
	Shaji Mathew Shaji Mathew is the Practice Administrator and the Technology Expert at Creekview Family Dentistry. Shaji received his B.S. in Computer Science from the University of Texas at Dallas. Before taking charge of the practice, Shaji was working as Information Systems Specialist for the U.S. Postal Service. Shaji received recognition from the President’s Council on Year 2000 Conversion, while working on the Y2K project for the U.S. Postal Service. Shaji is married to Dr. Mathew and they have a 19 year old son Joshua.
